Online Registration and Consent Forms
Types of Registration
Adult Unsupervised Climber - you are 18+ and competent to use the roped and bouldering areas
Adult Unsupervised Boulder Only - you are 18+ and are either a novice or experienced boulderer but do not have the safety skills to use the roped areas
Junior Unsupervised User - you are 14-17 years old and are competent in either:
- Bouldering only
- Top roping and bouldering
- Lead climbing, top rope climbing and bouldering
Guest Registration Record - you are a registered adult unsupervised user and wish to supervise guest climbers (adult novice or junior)
Supervised Consent - you are either U16 or over 16 and are using the wall either under the supervised of an adult unsuspervised user or a member of Perth College Climbing Centre Instructing Staff
External Instructor - you are a qualified climbing instructor who is bringing a group of over 2 people to Perth College Climbing Centre. We also require you to complete a group register form for each visit detailing the names of group members and any details for invoicing if required.
Adult Unsupervised Climbing and Bouldering
Adult Unsupervised Climbing and Bouldering
This form is for adults who are competent climbers and do not require any instruction. If you are new to UHI Perth Climbing Centre we will also ask you to give a quick practical demonstration of the safety skills required (fitting a harness, tying an appropriate knot, belaying and lowering a climber and securing a top rope fall).
Adult Unsupervised Bouldering Only
Adult Unsupervised Bouldering Only
This form is for adults who would like to register to use the bouldering wall only. Please complete this if you are a novice or experienced boulder only user. You will receive a boulder induction on arrival
Junior Unsupervised Boulder Only
Junior Unsupervised Boulder Only
This form is for Juniors (aged 14 – 17 years old) who wish to register as boulder only unsupervised users. This form must be completed by the Junior’s parent or guardian. You must be competent at bouldering and are required to undergo a Competency Assessment with a UHI Perth Climbing Centre staff member. This form is for Juniors (aged 14 – 17 years old) who wish to register as top rope and bouldering unsupervised users. This form must be completed by the Junior’s parent or guardian. You must be competent in top rope climbing and bouldering and are required to undergo a Competency Assessment with a UHI Perth Climbing Centre staff member. This form is for Juniors (aged 14 – 17 years old) who wish to register as lead climbing, top rope and bouldering unsupervised users. This form must be completed by the Junior’s parent or guardian. Supervising a Guest or Novice
Supervising a Guest or Novice
In order to supervise a guest you must already be registered unsupervsied user of the wall. You can sign in up to 2 guests per visit. All guests must complete a one off Supervised Consent Form (see below). On each visit you will be required to complete a Guest Registration Record form which details the Conditions for Supervising a Guest and records the date, supervising climber name and signature and guest name(s) and signature(s).

This form is for anyone under 16 years old, who is either registering as a Guest of a registered adult unsupervised user or taking part in an instructed session with UHI Perth Climbing Centre staff. Please note this form must be completed by the Parent or Legal Guardian of the Junior.

This form is for anyone 16 years + who is either registering as a Guest of a registered adult unsupervised user or taking part in an instructed session with UHI Perth Climbing Centre staff.
External Instructor Registration
External Instructor Registration
If this is your first visit we require you to complete an External Instructor Registration form and email a copy of your qualification, valid first aid and insurance to aswinfo.perth@uhi.ac.uk. On each visit we require you to complete a Group Register form which acts as evidence of group consent and a confirmation of who is under your responsibility on that day. If you require an invoice to be sent the details can also be put on this form, and an invoice will be sent out at the end of the month.
